Minor depression is diagnosed when symptoms have persisted for 2+ weeks as in the case of Michael, but do not meet the total evaluation for major depression (Moussavi et al., 2007). However, without treatment minor depression can develop into major depressive disorder (Nicholson et al. 2006). Furthermore, an estimated 50% of depressed individuals are not recognised in primary care (National Collaborating Centre for Mental Health, 2009). Diagnosis Depression can be reliably diagnosed and is covered by QS8. Diagnosis of depression is based on its severity and persistence, as well as the occurrence of other symptoms, as well as the extent of functional and social impairment (Kupfer, 1991). The National Health Service (NHS) has a number of models to aid clinicians to diagnose depression. As such, medical staff should be vigilant for mood changes, negativity, hopelessness and suicidal ideation, and increased support should be provided such as frequent contact (NICE, 2004). Evidence-Based Practice and Treatment There are a number of effective treatments for depression. The recommended treatment options for moderate to severe depression, as stated by the World Health Organisation (WHO), consists of psychosocial assistance in conjunction with antidepressant medication and/or psychotherapy, which includes CBT, interpersonal psychotherapy or problem-solving treatments. Initial treatment of depression often begins when the patient consults their GP, NICE (2011) states that a comprehensive assessment is required that is more than a symptom tally, but accounts for functional impairment or disability. To ensure this the GP will frequently give the patient a questionnaire to fill in for assessment which may use rating scales such as GAD, PHQ or Whooley. Michael was assessed as significant risk to himself, therefore he was referred to a specialist mental health service. NICE (2011) states that effective delivery of interventions for depression requires competent practitioners to deliver interventions, which may include psychosocial and psychological interventions. Michael’s treatment would consist of medication, however as he presents with suicidal ideation, medication toxicity as well as the quantities issued, should be assessed stringently (Simon et al., 2006). Antidepressants can be valuable in treating moderate to severe depression, but should not be the primary form of treatment for mild depression (WHO, 1992). The potential side-effects, addiction potential and importance of taking prescribed medication should be explained (Anderson, et al., 2008). Medication support should be provided for at least six months following remission of a depressive episode of (Jick et al., 2004). Michael should be monitored by nursing staff and he should be reassured that he can talk to them, although he must be told that staff have a duty to inform the doc tors of any concerns. It is important to remember to not offer false reassurance; problem-solving is the best treatment. If Michael is released into the community, he should be monitored at least weekly (WHO, 1992). NICE (2011) guidelines suggests that patients with continual sub-threshold depressive symptoms should be offered self-help guides on cognitive behaviour therapy (CBT) either manual or computerised, or structured group activity programmes. Michael (and his family, if he consents) should be advised of the expected symptoms, such as the potential for increased agitation and to be mindful of mood changes, negativity and suicidal ideation (Waraich et al., 2004). He should be offered some form of psychological therapy, especially to help with his feelings of loss, due to his failed romance and any other lifestyle problems that may be affecting him. These could include behavioural activation, cognitive-analytic therapy, cognitive behavioural therapy (CBT) and do-it-yourself CBT, group therapy, counselling (family or relationship), interpersonal therapy or psychodynamic psychotherapy/psychoanalysis (Simon et al., 2004). The use of psychosocial/psychological treatment and medication are benef icial in treating moderate to severe depression, such as Michael’s case. Competence frameworks should ensure the patient receives regular supervision, reviewed treatments with monitoring and evaluation of those treatments that may include video or audio tapes and external scrutiny (NICE, 2011) .Collaborative care ought to consist of case management, which should be administered by a senior mental health professional. Care of depressed individuals also requires close cooperation between primary and secondary health services and/or specialist mental health services. Finally, long-term coordination of care and good follow-up with Michael will reduce the risks associated with recurrence of depression. I wouldn’ t no what to do besides just sit there and watch. After the eight weeks were up we went to the doctor and got the brace off. After about eight weeks of physical therapy the pains began to come back in the right leg. We went back to the doctor and he said we could cast it again. I told him that I didn’t want to cast it again I just wanted to go straight to the problem. He disagreed so we decided to go get some other doctors opinions. After about twelve other doctors, most of them thought surgery would be the best way to deal with the problem. The doctor that was going to do the surgery was Dr. Crider. He wasn’t truly experienced in the knee area but we felt he understood the problem the most. Surgery day came and I was nervous. I never had surgery before and I remember always asking my dad the chances of dying. He told me not to worry and that everything would be okay. The anesthesia was actually fun. They put a gas mask on and I remember hearing funny sounds and seeing funny. When I woke up I kept falling back asleep. I wanted to wake up but the anesthesia kept putting me back to sleep. It was back to school after about a week and back to the sitting in at lunch with nothing to do. I pretty much had to deal with it and as time went by I became more and more bored and depressed. Halfway through the school year, I was able to start walking again. Things seemed fine. I was playing sports and going out during lunch with the other kids. But as time went by my left knee began to hurt. We went back to Dr. Crider and he said that we would need an MRI of the left knee. As in the right knee I had Osteochondritis Deccecans along with a Plica. By definition a Plica is the synovial tissue most prone to irritation and injury. When the knee is flexed, the plica is exposed to direct trauma, and it may also be injured in overuse syndromes. When the plica becomes irritated and inflamed, the condition called plica syndrome results. We once again had to do surgery. I just wanted these problems to go away. After the fun anesthesia and the eight weeks back in a wheel chair I was able to walk again. I was up and playing sports. I was on all the teams including Soccer and Tennis and my legs felt great. Two years had passed and I felt no problems. It was the summer of eighth grade and I was in camp. It was twenty four seven sports and fitness. The pain in my knees were back. I heard clicking noises when bending my knee and it would hurt to bend and straighten my knee. I knew what it was and I became very angry. It was going to be my first year in high school and I was going to have to spend in crutches and a wheel chair. I didn’t want to sit in at lunch, get bumped around which could re-injur my leg or get called cripple by kids in school. The new doctor named Dr. Watnik said it would probably be best to get home schooled. I was angry that I would miss much of the school year but I was also happy that I could play games and wake up late. After the MRIs on my knees he saw the Osteochondritis Deccecans in both knees and a plica in the left knee. After the first surgery it was really painful for the first couple of days, but as time passed I had become interested in the business world. I took about seven hundred dollars out of my bank account and began day trading on Scottrade. I would read all the business books and learn a lot about the business world. Time flew by and the second surgery was coming up. I was beginning to walk again, and in physical therapy we were working double time so that my right knee would be strong enough to support me after I had surgery on the left. The surgery was done and the pain returned. I tried not to think about it so much so I would pay attention to the business world a lot. I began to watch channel twenty four a lot and I learned a lot. As timed went by I found myself back in school and back to waking up at six thirty everyday. I was excited and happy to be with my friends again. Since I was in high school I was able to leave school grounds for lunch and that was something that I had never experienced. I had more freedom and I enjoyed it. Like my mom would always tell me, â€Å"from every bad thing comes something good.† Yes, I have knee problems but I am making money and I have found something I want to pursue for the rest of my life. I am walking again and hopefully the problem will never come back.

Roman Gladiators vs. the Gladiator Movie In May 2000,  Gladiator  opened in movie theaters. Maximus Decimus Meridius (Russell Crowe) is a successful general from the Battle of the Danube under Marcus Aurelius (Richard Harris). Commodus (Joaquin Phoenix), son of Marcus Aurelius, condemns Meridius to probable death by sending him into the gladiatorial arena. Commodus isnt merely sending to an uncertain death the general he perceives as a threat to his throne. The new emperor himself enters the arena to ensure Meridius permanent  end. If the plot seems a bit far-fetched, its not- at least in the most obvious way, because Commodus and probably another half dozen emperors did indeed set foot in the arena. Emperor Gladiators The adulation of the crowds has to be among the most compelling reasons to become a gladiator. At first, gladiators were slaves, criminals condemned to death, and war prisoners. In time, free men volunteered to become gladiators. Brooklyn Colleges Roger Dunkle says it has been estimated that by the end of the Republic, half the gladiators were volunteers. There were even women gladiators. That Emperor Septimius Severus banned female gladiators suggests that by the beginning of the third century A.D., there was a sizable number of such Amazons. Two of the mad emperors, Caligula and Commodus, appeared as gladiators in the arena. Seven other emperors who werent demented, including Titus and Hadrian, either trained as gladiators or fought in the arena. The Gladiator Was Honored but Unrespectable Anyone who became a gladiator was, by definition, infamis (whence: infamy), not respectable, and beneath the law. Barbara F. McManus says gladiators had to swear an oath (sacramentum gladiatorium): â€Å"I will endure to be burned, to be bound, to be beaten, and to be killed by the sword.†Ã‚  This consigned the gladiator to possible death, but also conferred honor, much like that of a soldier. Not only was there honor for a gladiator, but there were adoring crowds, and, sometimes there was wealth (victors were paid with a laurel, monetary payment, and donations from the crowd) and a life of leisure. Some gladiators may have fought no more than two or three times a year and may have won their freedom within very few years. Because of the financial incentive, free men and even aristocrats who, having squandered their inheritance had no other comfortable means of support, would voluntarily become gladiators. At the end of his service, a freed gladiator (as a token, he received a rudis), could teach other gladiators or a he could become a freelance bodyguard. The plot is familiar: In todays movies, the ex-boxer, having survived dozens of bloody KOs with only a few disfigurements, becomes a manager or trainer at a boxing school. Some popular sports figures become sportscasters. occasionally, they become television or movie personalities or even politicians. Political Gladiator Fights An editor is a person who gives something forth into the public, like a public game. In the Republic, the Editores were politicians who, wishing to curry public favor, would put on fights between gladiators and animal shows. Today, municipalities build stadiums with tax dollars, a burden shared rather than being shouldered by a benefactor. The person with the status of the editor may be the owner of the sports team. Clinical Computer Standards of Student Restorative Clinic

Clinical Computer Standards of Student Restorative Clinic Audit to assess the clinical computer standards of the student restorative clinic in the University of Manchester dental hospital Introduction: The use of computers in clinics is a useful tool for clinicians, however it can create challenges for their users with regards to cross infection control and patient data protection. The GDC (General Dental Council) and CMFT (Central Manchester Foundation Trust) have set standards for cross infection control and handling patient data. These standards were used to assess the clinical computer use on the ground floor restorative clinic in the University of Manchester dental hospital. Method: Clinical computers were assessed and data collected over the period of two weeks on Mondays, Wednesdays and Fridays. The computers were assessed using a data collection sheet. Results were compiled and analysed. Results: 89% of computers met the standards for data protection, 57% of the computers met the standards for cross infection control. Conclusion: The results show that standards for cross infection control for the computers on the clinic were particularly poor. They also showed that standards for data protection are not being met by some users of computers in the clinic. Action plan: Students and staff on the student restorative clinic should be made aware of results and recommendations should be implemented. A repeat audit should be undertaken in 12 months to monitor any improvement. Background The use of clinical computers in the dental hospital has a significant role in patient care. With the introduction of software to handle patient notes being introduced in September 2014 this is even more relevant than ever before. Now that students use the computers to record all their clinical notes, there is a larger amount of patient data being stored on them and the computers are being used much more frequently. Whilst the use of computers for clinical purposes can be a useful tool for clinicians, it also poses new challenges to those using them. Personal information of patients should be protected at all times and this means storing records securely where they will not be seen by other patients, unauthorised healthcare staff or members of the public (GDC, 2005) . Appropriate cross infection control should also be maintained when using computers on clinic. Cross infection control precautions are necessary to ensure the safety of patients and healthcare personnel (NHS, 2010). Therefore staff and students need to be aware of the proper procedures to follow when using the computers as part of their clinical practices. The clinical computers on the ground floor restorative clinics are situated within the clinical bays and are accessible to students and staff during and after treatment. Access to these computers is restricted through a password log-in providing they have been logged out of. Many of these computers have patient data freely accessible once the computers have been logged into. Currently there is no specific instruction to students to log out of the machines after use. The computers on the clinic use a conventional keyboard and mouse to operate. This creates a potential problem for cross infection as the computers are often used during treatment of patients. Aims and objectives The aim of this audit is to ensure that CMFT and GDC standards for cross infection control and patient confidentiality are adhered to in the dental hospital ground floor clinic with respect to clinical computer use. The objective of this audit is to assess the quality of cross infection control procedures and patient confidentiality with regards to clinical computer use and to devise implementations which can be made to improve practices and meet the standards set. Standards In order to assess the use of the clinical computers within the restorative clinic I have used policies and standards set by the CMFT and GDC Nationally. The CMFT have policies on cross infection and patient data protection available here: http://www.cmft.nhs.uk/your-trust/freedom-of-information/our-policies-and-procedures The policy document ‘Records Management: NHS Code of Practice’ outlines that equipment used to store records should provide storage that is safe and secure from unauthorised access and which meets health and safety regulations. Method The audit was carried out retrospectively. A total of 45 Computers on the ground floor restorative clinic were assessed on Monday, Wednesday and Friday afternoon for 2 weeks. Computers were assessed using a data collection sheet (appendix 1). Data from the collection sheet was compiled and analysed. The data collection sheet gathered information for the following categories: Cross infection control Computer hardware which must be touched for use i.e. Keyboards and mice were checked for appropriate barrier guard protection and cross infection control procedures. Computer keyboards must have barrier protection as appropriate cross infection control cannot be achieved by wiping with disinfectant due to the structure of the keyboards being used. The mice which are used can be wiped and so students were asked if they had wiped the mouse after use. Students were also asked if they had wiped the plastic cover of the keyboards. Keyboards which did not have barrier protection and those which did and were not wiped were deemed to not meet the standards of the CMFT. Mice which were not wiped with disinfectant were also deemed to not meet the standards of the CMFT. Data protection Computers were assessed to see if the users had logged off after use. Computers were also assessed to see if there was personally identifiable patient data accessible without restriction. Any computers with unrestricted patient data were deemed to not meet the standards set by the GDC and CMFT. Results The results from the collection sheet from all 6 days were compiled to give an average over the two weeks which were converted into percentages represented in the graphs below. Cross Infection Control This graph shows the percentage of the computers which had a plastic barrier guard protection over the keyboard at the end of the sessions. From the data collected an average of 24% of computers in the ground floor restorative clinic had no barrier protection during the clinical sessions. Of the computers which had barrier protection I asked the students from these bays if they had wiped the plastic barrier guard with disinfectant, the results are shown below. The graph shows that on average over the two weeks 18% of the keyboards with barrier protection were not wiped with disinfectant at the end of the clinical session. All of the mice for the clinical computers in ground floor restorative have no barrier protection and so students were asked if they had wiped the mice at the end of the session, the results of which are shown below. The data collected showed that on average over the two weeks 21% of students had not wiped down the mice with disinfectant after using them. Computers which did not have barrier protection for the keyboard and those which did but were not wiped as well as those with mice that weren’t wiped were considered to have inappropriate cross infection control. The total results represented as a percentage in the chart below. The data collected showed that in total 43% of computers being used in the ground floor restorative clinic were not meeting CMFT standards for cross infection control. Data Protection This graph shows the percentage of computers which were logged out of at the end of the clinical sessions over the two weeks. The data collected shows that 52% of computers on ground floor restorative were not logged out of at the end of the session creating a potential data protection issue. Computers were also assessed to see if any personally identifiable patient data was freely accessible without restriction, results shown as a percentage in the graph below. The data collected showed that 11% of the computers on ground floor restorative clinic had personally identifiable patient data freely accessible without restriction and thus these computers were deemed to be unsecure in terms of patient data protection. Therefore these computers do not meet the standards set by the CMFT and GDC for patient confidentiality. Conclusion Using the guidelines set by the CMFT and GDC I have assessed the clinical computer standards of the ground floor restorative clinic with regards to cross infection control and data protection of patients. The results show that only 57% of computers in the ground floor restorative clinic meet the standards of cross infection control. This was mostly due to barrier guards on keyboards not being used but it was also the case that students were not using disinfectant wipes on both the keyboards and mice. 89% of the computers meet the standards for data protection. The remaining 11% was a result of patient data being accessible directly from logged in machines thus the information was not secured. The results show that there is significant improvement to be made in the practices of staff and students using the computers on clinic to better protect patient confidentiality and wellbeing. Therefore action should be taken and recommendations implemented to improve the clinical computer standa rds of the clinic. Recommendations and action plan The results of this audit should be discussed with students who use the ground floor restorative clinic. It would also be useful to discuss the results with clinical tutors and other healthcare staff on clinic who oversee work done by students and who also sometimes use the computers. The clinical induction period for students at the start of the year would be an appropriate time to give specific instruction to students regarding clinical computer use. The instruction should highlight the correct cross infection control procedures for the computers including mandatory use of a barrier guard for keyboards and use of disinfectant wiped for both the keyboard barrier and mice. This could be included with other cross infection control inductions such as hand washing and usage of the clinical units. Instruction should also be given to students to log out of machines after use. If 100% of student implement this recommendation then data protection issues regarding the clinical computers would be largely resolved. The audit should be repeated in 12 months’ time to investigate any if any improvements have been made following implementation of recommendations. In the â€Å"Suma Theologica,† by Saint Thomas Aquinas, Aquinas combines the doctrines of Christianity with Aristotelian philosophy, through the assertion that as a result of observation, utilization of the senses, and an empirical mindset, one can substantiate the existence of God. Aquinas holds the conception that in the quest for God’s existence, philosophy and theology are interwoven together and both play complementary roles. He infuses numerous theological doctrines, mainly from Saint Augustine, alongside Aristotelian ideals such as, the first mover, to corroborate his argument for the existence of God. Instead of basing his argument on human reason and rationality like his predecessor, Saint Anselm of Canterbury, Aquinas holds that it is†¦show more content†¦Movement, brought upon by the action of another, reduces an object from a state of potency to act, because an object cannot change itself from stationary state to actual movement. For this change to occur, the object that is triggering this movement must itself be in a state of actuality (Aquinas 25). As a result, it is impossible for an object to dwell in a state of both actuality and potentiality, therefore, â€Å"whatever is moved must be moved by another,† that is it should move itself (Aquinas 25). From this, Aquinas draws the conclusion that an infinite regress is impossible because there is no beginning and no end and therefore there would be no first mover or no other subsequent movers.
